My care provider suggested hormone replacement therapy (HRT) to treat these symptoms. However, she recalled that in 2002, the Women’s Health Initiative (WHI) reported that of 10,000 women-years, HRT produced 18 more women with blood clots, eight more with stroke, and seven more with heart attacks than those not on HRT. I have a family history of blood clots. Is this treatment appropriate for me?

Extrapolating from the experiences of athletes who get exercise‐induced cramps and people with peripheral vascular disease who get activity‐related cramps, one would assume that  muscle cramps must represent muscle fatigue, electrolyte imbalance, or tissue ischemia. Yet,  many muscle cramps occur temporally distant from any physical exertion and often when  hydration is not an issue. And for many athletes, hydration or electrolyte solutions have failed to  prevent or adequately treat their occurrence.

52e26dff-3678-4913-8f8d-b85220fe7b82 <![CDATA[Metabolic Syndrome and the Role of Estrogen]]> Metabolic syndrome represents a cluster of adverse biologic events characterized by alterations in lipids, elevated blood sugar, and increased cardiovascular risk.  According to the National Cholesterol Education Program’s Adult Treatment Panel 111 Report, metabolic syndrome may be diagnosed when three of the following criteria are present: waist circumference greater than 88 cm, HDL-C less than 50 mg/dL, triglycerides greater than 150 mg/dL, blood pressure above 130/85 mmHg, or fasting blood sugar over 110 mg/dL. The greatest health risk of metabolic syndrome is cardiovascular disease, but this risk also is age related.  Sixty percent of postmenopausal women are affected by metabolic syndrome, whereas only 22% of the general population meet these criteria. These differences in prevalence underscore the dramatic physiologic changes that occur as a woman enters menopause.

Our bodies efficiently make our natural hormones. Cholesterol from our diet is converted into a family of progesterones, which then become our androgens, such as testosterone, androstenedione, and dehydroepiandrosterone (DHEA). Androgens are important since they are the substrate for all of the estrogens in our body. The ovaries alone convert testosterone to estradiol (E2), the most powerful of the estrogens. Fat cells can convert androstenedione to other weaker estrogens, including estrone (E1), only 40% as active as estradiol, and estriol (E3), only 10% as active as estradiol. Since menopausal symptoms seem to arise with falling estrogen levels, estrogen has been sought as treatment of these symptoms.

Yet, there are three types of breast cancer: ductal (80%), lobular (10%), and inflammatory (1% to 5%), all of which have different receptor characteristics that guide treatment. Approximately 75% of breast cancer is estrogen-receptor (ER) positive. However, there are conditions in which estrogen both fuels and, at other times, kills ER-positive breast cancer cells.  One type of breast cancer does not even recognize estrogen at all. It is estimated that in 2015, there will be 231,840 new cases of breast cancer and 60,290 cases of breast carcinoma in situ. And it is predicted that 40,290 women will die of this disease (cancer.org). Understanding the types of breast cancer offers promise for improved surveillance, prevention, and more effective treatments.

c22f1d4b-7315-4caa-922b-9b4e4523c474 <![CDATA[Hot Flash Connection to Puberty]]> Puberty is a dynamic process that occurs as young women emerge into their reproductive lives.  The prepubertal process begins in the hypothalamus.  There, gonadotrophin-releasing hormone neurons secrete gonadotrophin-releasing hormone (GnRH).  Gonadotrophin-releasing hormone, in turn, enters the portal system in pulses, stimulating the pituitary to produce luteinizing hormone (LH) and follicle stimulating hormone (FSH), both of which act on the maturing ovary to initiate the production of estrogens and androgens and then progesterone once ovulation occurs.  Once produced, rising levels of estrogen communicate back to the hypothalamus to slow the process.  But how does estrogen control this feedback process since there are no estrogen receptors on the GnRH neurons?  And what does this have to do with menopausal hot flashes?

6011552d-9fb0-42d5-bb02-b459cab8c623 <![CDATA[Update on Timing of Hormone Replacement Therapy in Menopause]]> In 1964, Dr. Robert Wilson, a gynecologist in New York City, advocated in his book Feminine Forever that all menopausal women should receive hormone replacement therapy (HRT).  The outrage of the feminist movement led the National Institutes of Health (NIH) to fund the Women’s Health Initiative (WHI) in 1991 to determine if HRT could safely protect women’s cardiovascular and bone health.  In July 2002, the trial was prematurely canceled due to findings of a statistical increase in both breast cancer and stroke.  Based on these findings and the resultant publicity, millions of women stopped their HRT, and doctors stopped prescribing HRT.  As a result of this precipitous change, the majority of menopausal women have gone without HRT for more than a decade.

Estradiol accomplishes this by depressing the ability of your fat cells and immune cells to produce a number of inflammatory proteins that are linked to many of the menopausal symptoms women describe. Even in the several years leading up to that one year without a menstrual period (a window of time called the Menopause Transition), fluctuations in ovarian production of estradiol cause the release of these inflammatory proteins. These fluctuations explain why mood swings, hot flashes, skin changes, and loss of libido may be encountered, even as the menstrual periods still are occurring.

After opening in 2001 in a 76‐seat theater in Orlando, Florida, it now shows worldwide and incorporates 25 popular songs from the 1960s to the 1980s. With clever word arrangements provided by four outstanding vocalists, it addresses most of the common personal menopausal issues women struggle to understand. Everyone who has had the opportunity to see this musical knows how brilliant it is. Yet, as I looked around the room, I was impressed by the large number of men in the audience. And with musical humor, this singular entertainment event has become the poster child of a major change that is occurring in our society; the public conversation of a one‐time forbidden topic.

3f0f6435-8b4c-483a-9b14-7fbd9dfd8a95 <![CDATA[Was The Women's Health Initiative Good Or Bad For Women's Health?]]> Most women are familiar with the Women’s Health Initiative (WHI), the largest randomized controlled trial to date, sponsored by the National Institute of Health (NIH) to evaluate the role of hormone therapy in menopause to protect cardiovascular and bone health. Begun in 1991 as a proposed 15‐year study, women in menopause with a uterus were randomized to take orally either a placebo or PremPro®, a combination of Premarin®, a conjugated equine estrogen (CEE), and medroxyprogesterone, a synthetic version of progesterone. Women with a hysterectomy were given either CEE alone or placebo. In part, this $725 million study was intended to resolve the controversy over whether menopause should be embraced as a natural transition in life, a position taken by the feminist movement at the time, or, as proposed by such books as Feminine Forever (Pocket Books, NY, 1968), that menopause was a hormone deficiency totally preventable with hormone therapy.

Nonetheless, the gynecologic impact of these treatments is significant. In one study, 42% of breast cancer survivors experienced vaginal dryness, 38% reported that intercourse was painful, and 64% felt loss of libido. For many women, these changes affect her relationship with her partner directly, thereby increasing her risk of depression. Furthermore, medical treatment for depression can accelerate loss of sexual interest and further endanger the relationship. In one series, when 610 breast cancer survivors with normal sexual activity were given antidepressants, 57% experienced loss of libido.

What are these bursting moments of hot flashes, why do my moods swing so widely, and what are these new awkward aches? Why does my doctor talk to me about my cardiac risks when fear of breast cancer is my mountain to climb, and what are my options for management of these problems? If my doctor talks about hormonal management, I am really confused regarding what are bioidentical hormones, should I go on hormones and for how long, and what are the differences between the estrogen patch and the pill?”
