Diabetes Education
Diabetes—it can be a scary word, but if you or someone you know has been diagnosed with diabetes or pre-diabetes, help is available at St. James Hospital (SJH) offering diabetes education and support programs to meet the health care needs in our community.
The Diabetes Self-Management Program is designed to help individuals and their families learn how to better manage their disease and lead a better quality of life. The program includes:
- Individual counseling
- FREE group classes
- Continuous Glucose Monitoring screenings
- Insulin Pump starts and maintenance
- Ongoing collaboration with the patient’s primary care physician
In 2015, Noyes Health (Dansville, NY) began collaborating with St. James Hospital to provide Diabetes Self-Management services. Local patients can continue to access Diabetes services at SJH in Hornell.
The program, based at Noyes Memorial Hospital and offered at St. James Hospital, is staffed by Certified Diabetes Educators (CDE), and is recognized by the American Association of Diabetes Educators. Managing diabetes is a long-term commitment, and we are delighted to be able to continue providing this service to patients in Hornell.
